Prescription Drugs
Our plan covers prescription drugs and refills with a small copayment. Over-the-counter medicines with a written order from your doctor are available for a copayment for each medicine.
Maternity Care
Cover services for maternity care provided by a physician, nurse practitioner, hospital or birthing center. Prenatal care, postnatal care, delivery, and complications of pregnancy are also covered.
Preventive Care
Free Preventive Care like routine doctor exams, immunizations, and screenings to keep you healthy
Mental Health Services
Required care is 100% covered. This includes: Behavioral therapy. Substance abuse treatment. Medications.
Gym Membership Reimbursement
Cash Reimbursement ($200 every 6 months) for facility fees if member attains at least 50 visits.
Vision, Dental and Hearing
VISION - Covers eye exams and glasses
DENTAL -Cover cleanings, checkups and dental work
